Goals of gynecomastia treatment

Since there are different causes of gynecomastia, the doctor must distinguish between two forms when making a diagnosis: If hormonal changes are responsible for the abnormal growth of the mammary gland tissue, it is “real” gynecomastia. If, on the other hand, the enlarged breast can be traced back to pure fat deposits, one speaks of lipomastia (also pseudogynecomastia). This "wrong" form of gynecomastia occurs in overweight and obese people. Depending on the symptoms, the breast may be reduced in size using different methods.

How do you recognize gynecomastia?

A female breast growth in men is called gynecomastia. Gynecomastia can be unilateral or bilateral in the chest area. It is caused by a change in the balance between female sex hormones (estrogens) and the male sex hormone (testosterone). The proportion of the female messenger substance, which is always present in the man's body in small quantities, increases. The causes of gynecomastia are usually hormonal and can only be determined by a doctor.

Men who notice increased breast growth should see a doctor. Only the doctor can determine whether it is real or just pseudogynecomastia. In pseudogynecomastia, only fat is stored in the breast, usually caused by being overweight, while real gynecomastia often has hormonal causes.

What methods are there?

Which method is used to treat gynecomastia always depends on the type of development and the extent of the breast formation. Therefore, a detailed consultation about history and expectations as well as a physical examination is very important.


If hormonal changes lead to an increase in the glandular tissue, it is the "real" gynecomastia. In this case, the breast tissue usually has to be removed by surgery. To do this, a small incision is made on the edge of the nipple through which excess tissue and possibly body fat can be removed. In order to combat breast enlargement sustainably, subsequent hormone therapy may often be necessary.


If the enlargement of the breast can be traced back to pure fat deposits, it is the "wrong" gynecomastia, also called pseudogynecomastia or lipomastia. With this method, a liquid is injected through a small opening on the side of the chest muscle, which causes the fat cells to float up. The surgeon can then suction off the fat cells and excess tissue with a thin cannula. The minimally invasive procedure can be carried out on an outpatient basis and under local anesthesia. If the man is only anesthetized locally, he is given an additional sedative. This twilight sleep makes the man insensitive to pain.

With liposuction, too, it occasionally happens that excess tissue has to be removed, but only if the skin does not contract to the expected extent. If the breasts are very large, it may be necessary to tighten the skin afterwards.

Which gynecomastia treatment method is right for me?

First of all, the doctor must diagnose whether it is real gynecomastia or lipomastia. If the breast is enlarged due to fat deposits, the breast can be reduced with liposuction. If there is an increased amount of mammary gland tissue, the surgeon makes an incision on the lower edge of the areola during the operation and removes the excess tissue. Usually both interventions are combined with one another.

Can gynecomastia come back after surgery?

In rare cases, the male breasts can enlarge again after an operation. It is therefore important to rule out hormonal causes before the operation. The new growth also depends on how much glandular tissue or how many fat cells were removed during the operation. If an almost complete removal has been carried out, a renewed formation of the male breast is usually unlikely. After an operation, it is important to exercise and to eat a balanced diet.

When does surgery make sense for gynecomastia?

In principle, an operation on a man's bosom makes sense if drug and hormone therapies are unsuccessful and if the quality of life is clearly limited by psychological distress or physical limitations or complaints. An operation can eliminate the symptoms and strengthen male self-confidence.

What are the risks?

In general, surgical treatment of gynecomastia is considered to be safe and efficient with few complications. However, as with any other surgical procedure, risks cannot be completely ruled out. In addition to undesirable reactions to the anesthetic, there may be wound healing disorders or longer-lasting after-effects in individual cases, which can usually be compensated for by administering medication or small corrective interventions.
